‘She cries in her sleep’: Deeper crisis looms beneath devastation from Myanmar quake

“I hate earthquakes. Earthquakes took my mother and my aunt away,” five-year-old Khin Yadanar told the UN Children’s Fund (UNICEF), after both her mother and aunt were killed when a brick wall collapsed on them.

Around 6.5 million children were already in need of humanitarian assistance before the earthquake, which compounded existing vulnerabilities resulting from the brutal civil war between multiple armed opposition groups and the military junta which seized power in a February 2021 coup.

Families now face a further threat from flooding and landslides with the arrival of the monsoon season.

Midwives are lifelines

As health services collapsed after the earthquake, “women, especially pregnant mothers, were severely impacted,” said Yu Yu, a midwife in Mandalay, speaking to the UN’s reproductive health agency, UNFPA.

Amidst the chaos, midwives have emerged as frontline heroes. Undeterred by aftershocks and confronting both physical obstacles and emotional challenges, midwives provided hope and life-saving support.

Yu Yu notably recalls the case of one of her patients who suddenly became stranded, unable to reach any medical facilities as she went into labour following the quake.

Without hesitation, Yu Yu rushed to her side: “When I reached her, she was exhausted, overwhelmed by fear and financial insecurity following the earthquake,” she recalled.

On that day, Yu Yu saved both mother and child, as the baby’s umbilical cord had wrapped itself around the infant’s neck.

UNFPA has deployed mobile clinics to ensure that women and girls continue to receive essential healthcare and protection services.

Deep trauma

“She cries in her sleep, and I worry something inside her has broken,” said Thida, mother of eight-year-old Thiri, speaking to UNICEF.

Beneath the visible devastation of the earthquake lies a deeper crisis: the profound psychological trauma that young survivors carry.

“I was so scared. My heart was beating so fast – and all I could think about was my parrots and cats at home,” said Thurein Oo, a ten-year-old boy who was praying at a mosque when the tremor struck.

Across earthquake-affected areas, parents are witnessing similar signs of distress in their children – sudden anxiety, emotional withdrawal, and sleepless nights, say UN aid workers.

In response to this growing mental health crisis, UNICEF and its partners have mobilised to provide critical psychological support to affected communities.

Setting up child-friendly spaces, UNICEF aims to promote psychosocial wellbeing, build resilience, and restore a sense of normalcy to children’s routines. Through various activities such as drawing, children learn to cope with their trauma.

“I coloured a picture of my mother,” said Khin, who lost her mother in the quake. “I feel better when I draw.”

Although the physical rebuilding of homes and infrastructure will likely take years, the emotional and psychological toll the earthquake has had on children cannot be left untreated.

“I like coming here,” said Thurein, referring to one of these spaces. “I feel safe, and I made a new friend who also lost his home,” he added.

Proactive planning

While earthquakes are among the deadliest natural hazards, it is the collapse of buildings that causes the most devastating effects. As such, proactive disaster risk reduction – such as making structures earthquake-resistant – is essential to reducing deaths and economic losses.

Focusing on risk-sensitive urban development, UN-Habitat and the UN office for disaster risk reduction (UNDRR) are also working across Myanmar to mitigate the risks future earthquakes could pose.

While little can be done to prevent natural hazards such as earthquakes from occurring, much can be done to mitigate their effects.

As rebuilding efforts are underway, the UN is dedicated to “ensuring that each step we take makes the rebuilt areas stronger and more resilient than before,” said Anacláudia Rossbach, Executive Director of UN-Habitat.

Researchers uncover factors linked to optimal aging

What are the keys to “successful” or optimal aging? A new study followed more than 7000 middle aged and older Canadians for approximately three years to identify the factors linked to well-being as we age.

They found that those who were female, married, physically active and not obese and those who had never smoked, had higher incomes, and who did not have insomnia, heart disease or arthritis, were more likely to maintain excellent health across the study period and less likely to develop disabling cognitive, physical, or emotional problems.

As a baseline, the researchers selected participants who were in excellent health at the start of the approximately three-year period of study. This included the absence of memory problems or chronic disabling pain, freedom from any serious mental illness and absence of physical disabilities that limit daily activities — as well as the presence of adequate social support and high levels of happiness and life satisfaction.

“We were surprised and delighted to learn that more than 70% of our sample maintained their excellent state of health across the study period,” says the first author, Mabel Ho, a doctoral candidate at the University of Toronto’s Factor-Inwentash Faculty of Social Work (FIFSW) and the Institute of Life Course and Aging. “Our findings underline the importance of a strength-based rather than a deficit-based focus on aging and older adults. The media and research tend to ignore the positive and just focus on the problems.”

There was considerable variation in the prevalence of successful aging based on the respondents’ age at the beginning of the study. Three quarters of the respondents who were aged 55 to 64 at the start of the study period maintained excellent health throughout the study. Among those aged 80 and older, approximately half remained in excellent health.

“It is remarkable that half of those aged 80 and older maintained this extremely high bar of cognitive, physical, and emotional well-being across the three years of the study. This is wonderful news for older adults and their families who may anticipate that precipitous decline is inevitable for those aged 80 and older.”  says Mabel Ho. “By understanding factors associated with successful aging, we can work with older adults, families, practitioners, policymakers, and researchers to create an environment that supports a vibrant and healthy later life.”

Older adults who were obese were less likely to maintain good health in later life. Compared to older adults who were obese, those who had a normal weight were 24% more likely to age optimally.

“Our findings are in keeping with other studies which have found that obesity was related to a range of physical symptoms and cognitive problems and that physical activity also plays a key role in optimal aging,” says co-author David Burnes, Associate Professor at the University of Toronto’s FIFSW and a Canada Research Chair in Older Adult Mistreatment Prevention. “These findings highlight the importance of maintaining an appropriate weight and engaging in an active lifestyle throughout the life course”.

Income was also as an important factor. Only about half of those below the poverty line aged optimally compared to three-quarters of those living above the poverty line.

“Although our study does not provide information on why low income is important, it is possible that inadequate income causes stress and also restricts healthy choices such as optimal nutrition. Future research is needed to further explore this relationship,” says senior author Esme Fuller-Thomson, Director of the Institute for Life Course & Aging and Professor at the University of Toronto’s Factor-Inwentash Faculty of Social Work.

 

Lifestyle factors are associated with optimal health in later life. Older adults who never smoked were 46% more likely to maintain an excellent state of health compared to current smokers. Previous studies showed that quitting smoking in later life could improve survival statistics, pulmonary function, and quality of life; lower rates of coronary events, and reduce respiratory symptoms. The study found that former smokers did as well as those who had never smoked, underscoring that it is never too late to quit.

The study also found that engaging in physical activity was important in maintaining good health in later life. Older adults who engaged in moderate to strenuous physical activity were 35% to 45% more likely to age well, respectively.

The findings indicated that respondents who never or rarely experienced sleep problems at baseline were 29% more likely to maintain excellent health across the study.

“Clearly, good sleep is an important factor as we age. Sleep problems undermine cognitive, mental, and physical health. There is strong evidence that an intervention called cognitive-behavioral therapy for insomnia (CBT-I) is very helpful for people living with insomnia,” says Esme Fuller-Thomson.

Wearable sensors styled into t-shirts and face masks

Imperial researchers have embedded new low-cost sensors that monitor breathing, heart rate, and ammonia into t-shirts and face masks.

Potential applications range from monitoring exercise, sleep, and stress to diagnosing and monitoring disease through breath and vital signs.

Spun from a new Imperial-developed cotton-based conductive thread called PECOTEX, the sensors cost little to manufacture. Just $0.15 produces a metre of thread to seamlessly integrate more than ten sensors into clothing, and PECOTEX is compatible with industry-standard computerised embroidery machines.

First author of the research Fahad Alshabouna, PhD candidate at Imperial’s Department of Bioengineering, said: “The flexible medium of clothing means our sensors have a wide range of applications. They’re also relatively easy to produce which means we could scale up manufacturing and usher in a new generation of wearables in clothing.”

The research team embroidered the sensors into a face mask to monitor breathing, a t-shirt to monitor heart activity, and textiles to monitor gases like ammonia, a component of the breath that can be used to track liver and kidney function. The ammonia sensors were developed to test whether gas sensors could also be manufactured using embroidery.

Fahad added: “We demonstrated applications in monitoring cardiac activity and breathing, and sensing gases. Future potential applications include diagnosing and monitoring disease and treatment, monitoring the body during exercise, sleep, and stress, and use in batteries, heaters, anti-static clothing.”

Seamless sensors 

Wearable sensors, like those on smartwatches, let us continuously monitor our health and wellbeing non-invasively. Until now, however, there has been a lack of suitable conductive threads, which explains why wearable sensors seamlessly integrated into in clothing aren’t yet widely available.

Enter PECOTEX. Developed and spun into sensors by Imperial researchers, the material is machine washable, and is less breakable and more electrically conductive than commercially available silver-based conductive threads, meaning more layers can be added for to create complex types of sensor.[1]

Lead author Dr Firat Guder, also of the Department of Bioengineering, said: “PECOTEX is high-performing, strong, and adaptable to different needs. It’s readily scalable, meaning we can produce large volumes inexpensively using both domestic and industrial computerised embroidery machines.

“Our research opens up exciting possibilities for wearable sensors in everyday clothing. By monitoring breathing, heart rate, and gases, they can already be seamlessly integrated, and might even be able to help diagnose and monitor treatments of disease in the future.”

Next, the researchers will explore new application areas like energy storage, energy harvesting and biochemical sensing, as well as finding partners for commercialisation.

A scientific recipe that helps babies stop crying, calm down and sleep in bed

New research published in Current Biology on September 13 demonstrates the importance of carrying crying infants rather than simply holding them. Led by Kumi Kuroda at the RIKEN Center for Brain Science (CBS) in Japan, the study details how crying babies are physiologically affected by being held, carried, and laid down. The data yield a simple but cost-free and effective technique that increases the chance of getting a crying infant to calm down and sleep in bed.

Most parents know the occasional frustration and discomfort of dealing with a crying baby. For some, it’s a regular occurrence that affects the baby’s ability to sleep and stresses out the parents. What can you do in this situation? Kuroda and her team found a “Transport Response” in distressed mouse pups and human babies in which infants calm down when carried by their mothers. The response is a complex series of parallel biological processes that result in reduced crying and lower heart rates, which helps parents to transport the infants.

Based on the ‘Transport response’ in which distressed animals calm down when carried, the behavioral and physiological data in this study showed that when babies are crying a lot, walking for about 5 minutes, following by sitting for about 8 minutes should help calm them down and put them to sleep. The new study used a baby ECG machine and video cameras to systematically compare changes in heart-rate and behavior as mothers acted out activities that are commonly used to calm infants, including carrying, being pushed in a stroller, and holding while sitting. Data during these activities were recorded from babies that were crying, awake and calm, or sleeping. At each heartbeat, behavior was assessed as asleep, alert, or crying, and scored accordingly. This way the researchers could track changes in both behavior and physiology with sub-second precision.

The experiment led to a few important findings. First, as Kuroda explains, “walking for five minutes promoted sleep, but only for crying infants. Surprisingly, this effect was absent when babies were already calm beforehand.” Among the babies studied, all had stopped crying by the end of the five-minute walk and had reduced heart rates, and about half were asleep. Second, sitting and holding crying babies was not calming; heart rates tended to go up and crying persisted.

The heartbeat measure allowed the researchers to dissect the effect of each micro-activity as infants were handled. The researchers found that the babies were extremely sensitive to all movements by their mothers. For example, heartrates went up when mothers turned or when they stopped walking. The most significant event that disturbed the sleeping infants happened just when they became separated from their mothers.

Every mother has experienced the disappointment of having a finally sleeping baby wake up again after being put down. The researchers pinpointed the problem using the heartbeat data. “Although we did not predict it,” says Kuroda, “the key parameter for successful laydown of sleeping infants was the latency from sleep onset.” Babies often woke up if they were put down before they got about 8 minutes of sleep. Thus, based on the data, Kuroda recommends that when babies are crying too much and can’t sleep, mothers should carry them steadily for about 5 minutes with few abrupt movements, followed by about 8 minutes of sitting before laying them down for sleep.

Although this procedure does not address why some babies cry excessively and cannot sleep, it offers an immediate solution that can help parents of newborns. Additionally, the researchers recognize the usefulness of heartrate data in this age of wearable fitness devices. “We are developing a “baby-tech” wearable device with which parents can see the physiological states of their babies on their smartphoness in real-time,” says Kuroda. “Like science-based fitness training, we can do science-based parenting with these advances, and hopefully help babies to sleep and reduce parental stress caused by excessive infant crying.”

Insufficient sleep in teenagers leads to obesity: Study

Adolescents who sleep less than eight hours a night are more likely to be overweight or obese compared to their peers with sufficient sleep, said a new study presented at ESC Congress 2022.

Shorter sleepers were also more likely to have a combination of other unhealthy characteristics including excess fat around the middle, elevated blood pressure, and abnormal blood lipid and glucose levels, said the study by Jesús Martínez Gómez, a researcher at the Cardiovascular Health and Imaging Laboratory, Spanish National Centre for Cardiovascular Research (CNIC), Madrid, Spain.

“Our study shows that most teenagers do not get enough sleep and this is connected with excess weight and characteristics that promote weight gain, potentially setting them up for future problems,” said study author Gómez. “We are currently investigating whether poor sleep habits are related to excessive screen time, which could explain why older adolescents get even less sleep than younger ones.”

This study examined the association between sleep duration and health in 1,229 adolescents in the SI! Program for Secondary Schools trial in Spain. Participants had an average age of 12 years at baseline with equal numbers of boys and girls.

Sleep was measured for seven days with a wearable activity tracker three times in each participant at ages 12, 14 and 16 years. For optimal health, the American Academy of Sleep Medicine advises sleeping 9 to 12 hours a night for 6 to 12 year-olds and 8 to 10 hours for 13 to 18 year-olds.To simplify the analysis, the study used 8 hours or more as optimal. Participants were categorized as very short sleepers (less than 7 hours), short sleepers (7 to 8 hours), and optimal (8 hours or more).

Overweight and obesity were determined according to body mass index. The researchers calculated a continuous metabolic syndrome score ranging from negative (healthier) to positive (unhealthier) values that included waist circumference, blood pressure, and blood glucose and lipid levels.

At 12 years of age, only 34% of participants slept at least 8 hours a night, and this dropped to 23% and 19% at 14 and 16 years of age, respectively. Boys tended to get less sleep. Teenagers who got the most sleep also had better quality sleep, meaning they woke up less during the night and spent a higher proportion of the time in bed sleeping compared to those with shorter sleep. The prevalence of overweight/obesity was 27%, 24% and 21% at 12, 14 and 16 years of age, respectively.

Compared with optimal sleepers, overweight/obesity was 21% and 72% more likely in very short sleepers at 12 and 14 years, respectively. Short sleepers were 19% and 29% more likely to be overweight/obese compared with optimal sleepers at 12 and 14 years, respectively. Similarly, both very short and short sleepers had higher average metabolic syndrome scores at 12 and 14 years compared with optimal sleepers.

Mr. Martínez Gómez said: “The connections between insufficient sleep and adverse health were independent of energy intake and physical activity levels, indicating that sleep itself is important.”

1 in 3 older adults take something to help them sleep but many don’t talk to their doctors

1 in 12 people over age 65 take prescription sleep medications, which carry health risks for older people, U-M/AARP National Poll on Healthy Aging finds.

Sleep doesn’t come easily for nearly half of older Americans, and more than a third have resorted to some sort of medication to help them doze off at night, according to new results from the National Poll on Healthy Aging.

But most poll respondents said they hadn’t talked to their doctor about their sleep, even though more than a third said their sleep posed a problem. Half believe — incorrectly — that sleep problems just come naturally with age.

The poll was conducted by the University of Michigan Institute for Healthcare Policy and Innovation, and is sponsored by AARP and Michigan Medicine, U-M’s academic medical center.

Those who turn to medications may not realize that prescription, over-the-counter and even “natural” sleep aids carry health risks, especially for older adults, either alone or in combination with other substances. In fact, national guidelines strongly warn against prescription sleep medicine use by people over age 65.

Despite this, the nationally representative poll of people ages 65 to 80 finds that 8 percent of older people take prescription sleep medicine regularly or occasionally. Among those who report sleep troubles three or more nights a week, 23 percent use a prescription sleep aid. Most who use such drugs to help them sleep had been taking them for years. Manufacturers and the U.S. Food and Drug Administration say such drugs are only for short-term use.

Medication: not the only option

“Although sleep problems can happen at any age and for many reasons, they can’t be cured by taking a pill, either prescription, over-the-counter or herbal, no matter what the ads on TV say,” says poll director Preeti Malani, M.D., a U-M physician trained in geriatric medicine. “Some of these medications can create big concerns for older adults, from falls and memory issues to confusion and constipation,” even if they’re sold without a prescription.

“The first step for anyone having trouble sleeping on a regular basis should be to talk to a doctor about it,” she continues. “Our poll shows that nearly two-thirds of those who did so got helpful advice – but a large percentage of those with sleep problems simply weren’t talking about it.”

She notes that non-medication-based sleep habits are the first choice for improving sleep in older people. Sleep and health

In all, 46 percent of those polled had trouble falling asleep one or more nights a week. Fifteen percent of the poll respondents said they had trouble falling asleep three or more nights a week.

Other health conditions can contribute to sleep difficulties. Twenty-three percent of poll respondents who had trouble sleeping said it was because of pain. And 40 percent of those with frequent sleep problems said their overall health was fair or poor. Other reasons for sleep troubles included having to get up to use the bathroom at night, and worry or stress.

Insomnia and other irregular sleep patterns can interfere with daytime functioning, and are associated with memory issues, depression and an increased risk of falls and accidents. Even so, many said they didn’t see sleep issues as a health problem – in fact, this belief was the most common reason that poll respondents said they didn’t talk to their doctor about sleep.

This also highlights the need for doctors to ask their older patients about their sleep habits and what they’re doing to address any issues they may be having

“We know that sleep is a critical factor for overall health as we age, and this new research highlights sleep problems as both a significant health issue for older adults and an underacknowledged one both by patients and their providers,” says Alison Bryant, Ph.D., senior vice president of research for AARP. “We need to help people understand that lack of sleep is not just a natural part of aging.”

More about medication use

In all, 14 percent of the poll respondents said they regularly took a prescription sleep medication, prescription pain medication, OTC sleep aid or herbal supplement to help them sleep. Another 23 percent took one of these options occasionally; most of the occasional users said they chose OTC sleep aids.

The most recent Beers Criteria established by the American Geriatrics Society, which guides the use of medications among older people, gives a strong warning against use of prescription sleep drugs, which are sold under such names as Ambien, Lunesta and Sonata.

Meanwhile, even though OTC sleep aids can be purchased without a doctor’s guidance or prescription, they still carry health risks for older people, Malani notes. Most of them contain diphenhydramine, an antihistamine that can cause side effects such as confusion, urinary retention and constipation.

Among poll respondents with frequent sleep problems who took something occasionally to help them sleep, OTC sleep remedies were the most common choice. But among those with frequent sleep issues who took something on a regular basis to try to sleep, prescription sleep medications were the most common option, with 17 percent reporting use.

Use of melatonin and other herbal remedies may be perceived as safer, but less is known about their potential side effects and they are not subject to the FDA’s approval process for medications, says Malani. But any issue that prompts someone to buy an OTC or herbal remedy on a regular basis is something they should discuss with their doctor, she adds.

The poll results are based on answers from a nationally representative sample of 1,065 people ages 65 to 80, who answered a wide range of questions online. Questions were written, and data interpreted and compiled, by the IHPI team. Laptops and Internet access were provided to poll respondents who did not already have it.

Could insufficient sleep be adding centimeters to your waistline?

Adults in the UK who have poor sleep patterns are more likely to be overweight and obese and have poorer metabolic health, according to a new study.

The findings showed that people who were sleeping an average of six hours a night had a waist measurement that was 3 cm greater than individuals who were getting nine hours of sleep a night. And shorter sleepers were heavier too.

The results strengthen the evidence that insufficient sleep could contribute to the development of metabolic diseases such as diabetes-major health challenges facing the NHS.

The study – led by Dr Laura Hardie, Reader in Molecular Epidemiology at the University of Leeds – not only looked at the links between sleep duration, diet and weight, but also other indicators of overall metabolic health such as blood pressure, blood cholesterol, blood sugar, and thyroid function.

The study involved 1,615 adults who reported how long they slept and kept records of food intake. Participants had blood samples taken and their weight, waist circumference, and blood pressure recorded. The researchers looked at the associations between how long people were sleeping and these key biological parameters.

The research team, from the Leeds Institute of Cardiovascular and Metabolic Medicine and the School of Food Science and Nutrition, reported their findings in the journal PLOS ONE.

Greg Potter, one of the Leeds researchers, said “The number of people with obesity worldwide has more than doubled since 1980. Obesity contributes to the development of many diseases, most notably type 2 diabetes. Understanding why people gain weight has crucial implications for public health.”

Shorter sleep was also linked to reduced levels of HDL cholesterol in the participants’ blood-another factor that can cause health problems. HDL cholesterol is ‘good’ cholesterol that helps remove ‘bad’ fat from the circulation. In doing so, high HDL cholesterol levels protect against conditions such as heart disease.

Interestingly, the study did not find any relationship between shortened sleep and a less healthy diet – a fact that surprised the researchers. Other studies have suggested that shortened sleep can lead to poor dietary choices.

The research was a snapshot of the associations between sleep duration and measurements of metabolic health. It was not designed to assess the impact of chronic poor sleep over time, and whether that leads to disease.

Dr Laura Hardie, the study’s senior investigator, added “Because we found that adults who reported sleeping less than their peers were more likely to be overweight or obese, our findings highlight the importance of getting enough sleep. How much sleep we need differs between people, but the current consensus is that seven to nine hours is best for most adults.”

The findings add to the growing body of evidence showing just how important a good night’s sleep is to health.

Sleep or sex? How the fruit fly decides

Choosing between sex or sleep presents a behavioral quandary for many species, including the fruit fly. A multi-institution team has found that, in Drosophila at least, males and females deal with these competing imperatives in fundamentally different ways, they report July 28 in the journal Nature Communications.

“An organism can only do one thing at a time,” said corresponding author Michael Nitabach, professor of cellular and molecular physiology and professor of genetics at Yale. “What we have discovered is a neuronal connection that regulates the interplay between courtship and sleep.”

Nitabach — in collaboration with scientists from Janelia Research Campus of the Howard Hughes Medical Institute, Southeast University in China, and University of San Diego — investigated neuronal activity involved in both behaviors and found that sleep-deprived males showed little interest in courtship while a lack of shut-eye had no effect on the mating behavior of females. Sexually aroused males got little sleep, while aroused females slept more.

The male flies’ behavior is easily explained as an adaptive behavior, say the scientists: Falling asleep during sex is not a good way to pass on your genes. But, they wondered, why are females still receptive to male advances when sleepy?

It could be that females can’t afford to pass up an eligible suitor no matter how tired they are, Nitabach said.

“It appears that whichever behavior has the highest biological drive suppresses the other behavior,” he said.

In addition to identifying this sex-specific behavior, the collaborative team also revealed underlying functional connections between the distinct neural centers that mediate sex and sleep.

Nitabach said humans could possibly have a similar mechanism for adjudicating when the drives for sleep and sex collide.
